Overview
Create an automated intake workflow in the workflow management package that formalizes and automates the process of "intaking" FR/BR/UXR reports into the Kanban system. This workflow handles decision flow analysis, story/task creation, dependency wiring, and intake documentation updates.

Problem: In commit f092c1f (v0.2.11.9+3), 5,174 lines were accidentally removed from CHANGELOG.md, deleting 11 changelog entries (0.2.8.1+1 through 0.2.8.5+1, and 0.2.11.1+1 through 0.2.11.6+1). The commit message did not mention changelog pruning, suggesting this was accidental.
Investigation Objectives:

Problem: The enhanced validator (E2:S01:T06) now correctly detects Keep a Changelog format and identifies ~100+ ordering violations in CHANGELOG.md. While the validator is fixed, the CHANGELOG content itself needs to be corrected.

- Fix all ordering violations in CHANGELOG.md
- Ensure all entries are in correct Keep a Changelog order (newest first)
- Preserve all entry content, links, and references
- Document process improvements to prevent future violations
